To initiate Opiate Replacement (Methadone) Treatment for yourself or a loved one, the following is provided as a guide to the steps necessary to begin at ICCS. The person needing the services must:

  • Call our main number at (360) 652-9640 (6:00am - 2:00pm) and schedule a chemical dependency assessment
  • Bring to the assessment a picture ID & form of payment for treatment services (cash, medical coupon)

To transfer from another agency, all of the following information must be faxed to ICCS from your current treatment provider:

  • Signed Release Of Information (ROI) to allow ICCS to communicate with your current clinic
  • Complete CD admission assessment
  • UA record
  • Medical notes including all labs
  • Most current dose level
  • Most current tuberculosis (TB) test and results
  • Current financial status
  • Court mandated treatment information
  • Confirmation of compliant status in all areas of treatment
  • Reason for transfer
